ICE

Robin Weeks, 375th Medical Group utilization manager, is this week’s 375th Air Mobility Wing “Showcase on ICE.” Weeks is a key member of the Medical Management Team and a member of the Population Health Working Group at the 375th MDG. In this role, she coordinates appropriate care for 379 high-risk patients with complicated diagnoses. Through her medical management, innovation, and communication, she works tirelessly with patients, providers, disease managers, and case managers to better meet the needs of our patient population. Her attention to detail led to the coordination of countless out-of-area network referrals, averting breaks and delays in specialized care, while eliminating unnecessary appointments for patients. Due to her ceaseless efforts, the number of patients considered high consumers of medical services decreased from 1.6 percent to 0.5 percent of the total population, effectively reducing the number of patient visits to the Emergency Rooms, Urgent Care Centers, and the clinic, while providing overall better management of care and improving outcomes.
