Book school physicals with 375th Medical Group Published May 21, 2015 By Maj. Angela Yuhas 375th Medical Support Squadron SCOTT AIR FORCE BASE, Ill. -- The 2015-2016 school year is fast approaching and the 375th Medical Group has school and sports physical appointments available. Illinois law requires a school physical for children entering preschool, kindergarten, 6th and 9th grades. Additionally, Illinois requires up-to-date immunizations, eye examinations, and physicals for all students new to the state. The 375th Medical Group has automated the Illinois school/sports physical form. Therefore, there is no need to bring a copy to the appointment. Upon arrival, the parent/guardian will be asked to complete a questionnaire prior to the appointment. To expedite the check-in process, members can request this required questionnaire through MiCare at http://app.relayhealth.com. Staff will review and validate the questionnaire with the parent/guardian. Clinic personnel will provide a signed copy of the Illinois state form to give to the school. Please make a copy for records before giving one to the school. A parent or guardian must be present with the child during the physical. Many people will be booked during this time so we ask that members arrive 15 minutes prior to their appointment with all necessary forms. Immunizations will be available for those needing shots or verification of vaccinations. If a child's shot record is not in the Medical Group's computer (he or she has not received shots at an Air Force clinic before) members must bring a written copy of the shot record to receive vaccinations. Children requiring a school physical do not need an annual physical unless they are involved in a sport where a sports physical is required. Sports physical forms dated after June 1, 2015 are good for the 2015-2016 school year. Book a school/sports physical today by calling 256-WELL (9355).
