MDG clarifies school physical process Published May 29, 2008 By 375th Medical Group SCOTT AIR FORCE BASE,Ill. -- Editor's note: This is an update to an article in an earlier edition of the Command Post. This is the most current and accurate information. The 375th Medical Group has started offering annual school physicals for 2008. Illinois law requires a school physical for children entering pre-school, kindergarten, 5th and 9th grades, as well as all children participating in scholastic sporting events. Additionally, Illinois requires school physicals for all new students. Children not in the above categories do not need an annual school physical. Sports physicals are not a covered benefit under TRICARE and parents are encouraged to participate in school-sponsored physical programs if they are offered. Furthermore, parents who know they will be relocating to a new state and wish to obtain school physicals prior to leaving Scott Air Force Base can download the appropriate school physical form from the state they will be relocating to, and bring the form to their child's appointment. The 375th Medical Group has automated the Illinois school physical form. Therefore, there is no need to bring a copy to the appointment. Upon arrival, parents will be asked to complete a brief form with specific questions. The provider will review and validate the form with the child and/or parent. Clinic staff will print a copy of the school physical form, the provider will sign the form after the examination, and parents will be provided a copy. A parent or guardian must be present with the child for the duration of the physical. Due to the limited time to complete school physical exam and appropriate forms, acute or other non-related issues should be addressed by making a separate appointment with providers. Also, for sake of time, only the appointment related school physical form will be completed at the scheduled visit. Additional forms, to include summer camp, Boy Scout, childcare, etc., will be left with the provider and can be picked up after seven duty days. Parents should make school physical appointments with the clinic where their child is currently enrolled. If unsure of your child's primary care provider, you may inquire when scheduling your appointment. School physical appointments may be booked by calling 256-9355 or online at http://www.tricareonline.com. When first using TRICARE Online, parents must register the child on the site. Once registered, access the appointments section and select a "WELL School Physical" appointment.
