Primary care changes bring new name and service model Published Jan. 8, 2009 By Capt. Kevin Hammond 375th Medical Group Practice Manager SCOTT AIR FORCE BASE, Ill. -- The Primary Care Clinic at Scott has been selected as a test site for the Family Health Initiative; a new staffing and health care delivery model for family medicine. With this new model comes a new name: Scott Family Health Clinic. The new model is designed to permit greater flexibility in scheduling patient appointments, while allowing our family medicine providers better continuity with their patients. Our goal is to create an enjoyable and productive environment that promotes and delivers world class healthcare to our community. One way in which we hope to improve the care that we deliver is through consistent appointments with the same provider. It is unlikely that every appointment will be with the same provider due to urgent care needs and provider availability, but we will strive to make it so. Why is this important to you? Greater flexibility for scheduling appointments with your team providers can foster a better relationship consistent with the best medical practice benchmarks. Your provider will have a portion of every day set aside in order to manage more complex medical issues. This time will be managed by the medical staff, providing the opportunity to schedule longer appointments for more complex conditions. Your Family Health Team will also work hand-in-hand with a new team of nurses (Disease Management Group) whose primary focus is the management of chronic illnesses, such as diabetes. As a result, the provider team will collaborate with the disease management group to provide focus on your specific wellness and preventive medicine needs. This team approach will allow us to improve monitoring and managing ongoing diseases. The 375th Medical Group will implement the Family Health Initiative on Jan. 13 to begin the test of this new healthcare model. The clinic will have patient appointments available from 7 a.m. to 4:30 p.m.; however, the Medical Group will still be closed the second Thursday of every month for mandatory training from noon to 5 p.m. In addition, this change will not affect Pediatrics, Internal Medicine, Flight Medicine or the Belleville Family Medicine Clinic. These clinics will continue to provide high-quality medical services to their enrolled patient population. Scott has a long heritage of innovation, mission ownership and driving success.
