375th MDG to host school physical day

The 375th Medical Group will host its annual school physical day Aug. 5.

Illinois law requires a school physical for children entering pre-school, kindergarten, first, sixth and ninth grades, as well as all children participating in school sports.

Additionally, Illinois requires up-to-date immunizations, eye examinations and school physicals for all new students. Children not in the above categories do not need an annual school physical.

The 375th Medical Group has automated the Illinois school physical form. Therefore, there is no need to bring a copy to the appointment.

Furthermore, parents who know they will be relocating and want to complete their child's school physical before leaving Scott, should download the appropriate school physical form for the new location and bring it to the appointment.

Upon arrival, parents will be asked to complete a brief form with specific questions. The provider will review and validate the form with the child and parent. Clinic staff will print a copy of the school physical form, the provider will sign the form after the examination and a copy will be provided to the parent.

A parent or guardian must be present with the child during the physical.

Due to the limited time to complete school physical exams and appropriate forms, acute and other non-related issues should be addressed at a separate appointment.

Parents should make a school physical appointment with the clinic where their child is currently enrolled. Anyone unsure of their child's primary care manager should inquire when scheduling the appointment at 256-WELL (9355).
