2011 School Physical Day is Aug. 4

Scott Air Force Base, Ill. --  The 375th Medical Group will host its annual school/sports physical day Aug. 4.

Illinois law requires a school physical for children entering pre-school, kindergarten, first, sixth and ninth grades. Additionally, Illinois requires up-to-date immunizations, eye examinations, and school physicals for all new students.

Children not in the above categories do not need an annual school physical unless they are involved in sports that require a physical. Boy Scout camp physicals will not be addressed on this day.

The 375th MDG has automated the Illinois school/sports physical form. Therefore, there is no need to bring a copy with you to the appointment. Upon arrival, the parent will be asked to complete a brief form with specific questions. Staff will review and validate the form with the parent.

Clinic personnel will provide parents a signed copy of the Illinois state form to give to the school. Parents should make a copy for their records before giving one to the school. Also, Illinois High School Association or Illinois Elementary School Association forms will not be completed at this visit because the printed product parents receive at the end of the appointment is an acceptable substitute.

A parent or guardian must be present with the child during the physical. Dropping children off at the clinic to expedite the process is not allowed. Due to the limited time to complete children's physical exam and appropriate forms, acute and other non-related issues will be addressed by making a separate appointment with a primary care manager.

Many people will be booked each hour so we ask that you report in the hour in which you are scheduled. Expect to be in the clinic from 30 minutes to two hours.

In addition to school/sports physical day, the 375th MDG will be hosting a health fair in the auditorium located in the basement. Agencies providing information include, but are not limited to, the Base Fire Department, Health and Wellness Center, Base Library, Illinois Poison Control, Scott Credit Union, and fingerprinting by Security Forces.
